How to get images from SimonMed? ›

Email: Please email the completed “Authorization to Release Protected Health Information” and a photo copy of your valid ID to MedRecReq@SimonMed.com. All requests will be expedited as quickly as possible (no longer than 30 days).

SimonMed accepts major credit cards, personal checks, or cash. Does SimonMed accept Care Credit or HSA cards? We do accept Care Credit and HSA or HRA cards.
